2、成5组,每组10个,分别使用ProTaper Universal(PTU组)、ProTaper Gold(PTG组)、Reciproc Blue(RB组)、One Curve(OC组)和Rotate(R组)进行预备,并记录预备时间。拍摄树脂根管预备前后的墨水染色图像,并通过计算机图像分析软件进行重叠,测量分析根管偏移量及中心定位能力等指标。结果5组预备时间总的比较差异有统计学意义(F=2230.958,P 0.05),其中PTU组预备时间最长,RB组的预备时间最短。在距离根尖孔1、2、3、8 mm处,PTU组的根管偏移量大于其他组,差异均有统计学意义(均P 0.05);在距离根尖孔4、5 mm
3、处,PTG组、OC组和R组的根管偏移量小于其他组,差异均有统计学意义(均P 0.05);在距离根尖孔6、7 mm处,OC组、R组的根管偏移量小于其他组,差异均有统计学意义(均P 0.05)。结论5种机用镍钛器械均可快速高效地预备弯曲根管,且可较好地保持重度弯曲根管的原始形态。在根尖段,与ProTaper Universal相比,其他4种器械均表现出良好的中心定位能力;在根管弯曲段,ProTaperGold、One Curve和Rotate的中心定位能力相对优于其他2种器械。关键词:镍钛器械;根管预备;弯曲根管;根管偏移;中心定位能力中图分类号:R78文献标志码:AComparison of s

10、 apicalforamen,the canal transportation of groups OC andR was smaller than that of the other groups,and the87Chinese Journal of Practical Stomatology Jan.2023 Vol.16 No.1在弯曲根管预备过程中,如何在保持根管原始形态的基础上实现连续的锥形形状仍然是临床上面临的巨大挑战1。由于受到根管解剖结构的复杂性和根管预备器械的局限性等多种因素的影响,机械预备过程中容易发生根管偏移,最终影响根管治疗的预后2。因此,根管预备器械具有良好的成形能
11、力显得尤为重要3。本研究以树脂模拟弯曲根管为研究对象,观察新型机用镍钛器械ProTaper Gold、Reciproc Blue、One Curve 和 Rotate在重度弯曲根管中的成形能力,并与经典的镍钛器械ProTaper Universal比较,为新型镍钛器械的临床应用提供参考。1材料与方法1.1材料与设备单弯树脂模拟根管、ReciprocBlue、Rotate(VDW,德国);根管冲洗器(27G,河南省宇安医疗科技开发有限公司);DTA凝胶(Well-prep,韩国);蒸馏水;红黑墨水各1瓶 上海英雄(集团)有限公司;10号K锉、ProTaper Universal、ProTaper
12、 Gold、X-Smart马达(Dentsply,瑞士)、OneCurve(康特,瑞士)、牙科显微镜(F12型,Leica,德国)。1.2研究方法1.2.1研究分组取50个标准的单弯树脂模拟根管,根尖孔直径均为0.15 mm,锥度为0.02,工作长度为16 mm,弯曲角度约为40(Schneider法测量)。随机分为5组,每组10个。分别用ProTaperUniversal(PTU组)、ProTaper Gold(PTG组)、Reciproc Blue(RB组)、One Curve(OC组)、Rotate(R组)进行根管预备。1.2.2预备前图像采集将单弯树脂模拟根管用“+”标记进行定位,用1
13、0号K锉探查根管并初步建立直线通路,蒸馏水冲洗,待自然干燥后灌注黑色墨水。将树脂模块固定于自制固定装置上并在旁边固定刻度尺,以便于后期图像重叠处理及比例换算。使用单反数码相机垂直于根管中轴采集预备前图像并储存,图像均储存为JPEG格式,记录预备前根管的原始形态,然后将根管内的墨水冲洗干净并干燥。1.2.3根管预备由同一操作者分别使用5种镍钛器械并严格遵循厂家推荐的扭矩和转速(表1)对5组树脂根管进行预备,终末工作宽度均为25号。预备过程中以17%EDTA凝胶作为根管润滑剂。完成一次预备后,用75%乙醇棉球擦拭器械,更换器械的过程中使用2 mL蒸馏水冲洗,并用10号K锉疏通根管后再用2 mL蒸馏
14、水冲洗以防止根尖碎屑堵塞。在牙科显微镜下观察器械有无形变、裂纹等,每套镍钛器械预备5个根管后弃用。在进行根管预备的同时,助手记录每支器械在根管内预备的时间。difference was statistically significant(P 0.05).ConclusionFive nickel titanium instruments can prepare curvedroot canals quickly and efficiently,and can keep the original shape of severely curved root canals.Compared with
